I will start with question #4.  The oldest info that I have found so
far in the archives is an "article written corporately by forty-six
teachers and school administrtors meeting as n academy at the
Ecumenical Institute: Chicago, using the methods of brainstorm and
gestalt designed to enable the uses of every insight present.  This
article represesnts an informed stance toward the situation in
education today. (August 1969)"

That doesn't answer your question.  But I will see what information I
can find about a "who" involved in creating curriculum.  As you know
most things were done corporately.
Then I'll see what  I can speak to your other questions.  Today I'm
involved in looking for South America HDP information.
