Wow!  Talk about one person's trash being another's treasure!  Sure glad I 
didn't throw it away. :)

David, if you're willing to scan this and put it on the web, that would be 
great.  That way everyone who wants it can access it.  I'll mail a copy to you 
in the next day or two.

Will you post something to the list when you've added it to the wiki site?

BTW all, this document does say something like "this is a working document; 
not for general distribution."  So it's possible it's not the final final 
thing.  I assume folks will still find it of interest.
