Right you are. A printed book starts with a PDF: download and print  
all or part of the PDF yourself at a nominal cost; order online or buy  
from a small inventory at near cost for the printing. (I'm thinking of  
lulu.com or similar on-demand printing for those who want a  
professionally printed hard copy.)
